Provider Credentials

All clinical medical staff are actively licensed in Illinois and nationally board certified. We operate a HIPAA complaint facility where confidentiality and privacy are highly regarded.

Do you accept insurance?

Not all services are insurance billable. The following services may be covered by your insurance carrier: routine health examinations, general medicine, telemedicine evaluations, health home services, massage and labs. Regenerative medicine, DOT, Immigration and travel vaccinations are not insurance reimbursable examinations and services.

Billing and Payments?

Insurance verification and authorization can be provided in advance of scheduled appointment. You must provide your insurance information for this to be done. You will be advised about insurance eligibility before services are rendered. Any out of pocket cost or payments will be collected prior to service being rendered. Payments can be made online or in person. We do not accept cash payments.

Medical and Drug Screenings?

Proof of identity with government or employment issued photo ID must be provided. All screenings are done on site by a medical provider. Testing is done by a licensed third party within 24 hours.

Cancellation and Rescheduling?

We understand that unexpected events can arise. You may reschedule an appointment up to 24 hours prior to your scheduled appointment time. Late cancellations will be charged $20.00. No service will be rendered until the late cancellation fee is paid.
